PHELPS Patricia 'Hazel' Passed away at Royal Berkshire Hospital on Wednesday 21st February 2018, aged 78 years. A loving mum and nan who will be sadly missed by all her family and friends. Funeral service will take place at Reading Crematorium, South Chapel on Monday 12th March 2018 at 2.30pm. Family flowers only please, donations in memory of Hazel made payable to the Berkshire Autistic Society may be sent c/o Cyril H Lovegrove, 21 School Road, Tilehurst, Reading, RG31 5AR, Tel 0118 945 2103.
