The 25-year-old was rested for Tuesday's win over Exeter City and his replacement, Jayden Wareham, scored inside 60 seconds.
However, the prolific forward scored in both meetings with the Pirates last season and returns from the start as Reading look to record three successive league wins for the first time since 2021.
Joel Pereira remains in goal behind an unchanged back four of Michael Craig; Tyler Bindon; Amadou Mbengue and Andre Garcia.
Lewis Wing sits deeper in a midfield trio alongside Harvey Knibbs and Ben Elliott, with Charlie Savage dropping to the bench.
Smith is flanked on either wing by Kelvin Ehibhatiomhan and Mamadi Camara, who starts in place of Chem Campbell.
Former Royal Clinton Mola starts for Matt Taylor's Rovers, having made 35 appearances last season before moving to the Memorial Stadium over the summer.
Experienced forward Scott Sinclair, who scored a hattrick against Reading for Swansea City in the 2011 Championship play-off final, also starts.
